the aim spot is based on numbers, you can use the arrows to assign the designated spot desired, I use hand spot that way I can miss a few shots and look legit.

As others have said, it is the menu item called "spot". I believe 0 is head, 1, is neck, 2 is spine, 3 is body, etc.. I had issues with this too at the beginning. Otherwise great cheat!
